Reactivation Care Centre – Church site adds 120 new hospital beds to the health system

After undergoing extensive renovations, the Reactivation Care Centre (RCC) – Church site opened four additional 30-bed units on May 5, 2019 with eligible patients from the four phase two hospitals – Southlake Regional Health Centre, St. Joseph’s Health Centre, Trillium Health Partners and William Osler Health System – moving into their respective units.

Similar to the model of care at the Reactivation Care Centre – Finch site, the RCC Church site will provide restorative care and help patients transition from hospital to the community. Benefits of the Reactivation Care Centre include specialized activation therapies in a setting designed to focus on the needs of the patient and support transition to home or long-term care. The Centre will act as a hub of specialized care focused on optimizing the patients’ strengths and abilities outside the acute care setting.

The Reactivation Care Centre – Church site is an extension of six Greater Toronto Area hospitals – Humber River Hospital, Southlake Regional Health Centre (Central LHIN), Sunnybrook Health Sciences Centre, St. Joseph’s Health Centre (Toronto Central LHIN), Trillium Health Partners (Mississauga Halton LHIN) and William Osler Health System (Central West LHIN). Each of the hospitals have their own unit that are staffed and operated as an extension of their hospital.

LHIN care coordinators work onsite at the Reactivation Care Centres, alongside hospital staff, to support patients and families with safe, timely discharges while improving access to care and reducing wait times.

Located at 200 Church Street, Toronto, the Reactivation Care Centre – Church site opened in December 2018 with 92 beds – 30 beds for Humber River Hospital and 62 beds for Sunnybrook Health Sciences Centre.
