The Central LHIN is committed to improving quality, access and appropriateness of care for people with musculoskeletal (MSK) conditions. One way we are working to improve care for MSK conditions is through the new MSK Central Intake, Assessment and Management model of care, which offers a centralized referral system for hip and knee arthroplasty and low back pain patients.

Rapid Access Clinics for Hip and Knee

All primary care physicians are required to fax referrals for primary hip and knee joint arthroplasty to a centralized fax number: 855-346-9138.

Rapid Access Clinics for Low Back Pain

Prior to completing your first referral, we ask that all Primary Care Providers complete an online onboarding and registration module. The module was created to familiarize providers with both the ISAEC model of care and the referral process. It includes information about red and yellow flags as well as low back assessment. This module takes only a few minutes to complete.
