In 2019, the government introduced Bill 74, The People’s Health Care Act. This legislation outlines requirements to integrate LHINs and other provincial health agencies into a single agency, Ontario Health, and supports the establishment of local Ontario Health Teams that connect health care providers and services around the needs of patients and families.
Core to the government’s plan for the creation of Ontario Health Teams is that, together, these teams will manage the delivery of care, and guide patients and families to navigate seamlessly through care transitions. Each team will be made up of a group local health care providers – including primary care, hospitals, home care, long-term care, mental health and addictions supports, and others – organized to work as a coordinated group.
